These terms and conditions (“Terms”) set forth the terms and conditions of the Referral Program (“Referral Program”) of Prudent Pet Insurance Agency, LLC (“Prudent Pet”). By participating in the Referral Program, you acknowledge that you have read, understand and agree to be bound by these Terms.

Eligible Participants

To be eligible for participating in the Referral Program, you must be eighteen (18) years of age or older, a legal United States resident, an active policy holder with Prudent Pet, and not be a resident of California, New Hampshire, Louisiana, Michigan, West Virginia, Georgia, South Dakota or any other state where participation is prohibited. All participation is void where prohibited.

How It Works:

If you are an eligible participant, you may visit www.prudentpet.com/customer-referral for inputting the email address or cell phone number for your friends, family members or other acquaintances that you choose to refer. By submitting your referral request, you are initiating and sending an email to your referral about Prudent Pet’s pet insurance. In doing so, you represent that you have all prior written and oral consent required by applicable laws and regulations from each referral for use of their email address and phone number in connection with the Referral Program.

If your referral completes a Prudent Pet application for pet insurance within 30 days following the sending of your email, the Reward (defined below) will be sent to you in accordance with these Terms. Completion of a Prudent Pet application for pet insurance means that your referral actually receives a pet insurance quote from Prudent Pet after completing an application with all required underwriting information. No purchase is necessary or required to qualify for any Reward or Donation.

Reward:

To qualify for a Reward, the referred person completing the application (applications with multiple pets will still be considered one referral) must provide the same email address and/or phone number that was utilized to send the applicable referral. You may not refer strangers, yourself or any other person who shares your email address, phone number or primary address. Neither you, your referrals or any other party may participate in the Referral Program using multiple or fake email addresses, phone numbers or other information.

The reward (“Reward”) for qualifying referrals will be a $15 Amazon.com gift card that will be electronically sent to you in accordance with these Terms. Additionally, and independent of the Reward, Prudent Pet will make a $5 donation (“Donation”) to a pet friendly charitable organization selected by Prudent Pet. Rewards will be sent within 30 days of a completed application. Donations will be made on a scheduled basis. All participants are limited to a maximum of $250 of Rewards per calendar year.

Prudent Pet reserves the right to refuse to pay any Reward or Donation resulting in connection with any fraud, suspicious activity or actions that are in violation of these Terms. The payment of all Rewards and Donations are subject to verification.

Limitations of Liability:
Prudent Pet and each of its respective affiliates, subsidiaries, parent corporations, and their respective officers, directors, shareholders, employees and agents shall not responsible for any incorrect or inaccurate information, theft, tampering, destruction, or unauthorized access to, or alteration of entries; printing or other errors; any entries or mail which are late, lost, delayed, incomplete, misdirected, stolen, mutilated, illegible, postage due or any combination of the above. Prudent Pet reserves the right to modify and change the Reward and Donation amounts, as well as suspend or terminate the Referral Program, at any time (provided, however, that qualifying Rewards and Donations will still be granted through the date and time of termination).

Release of Liability:

By participating, all participants and entrants agree to release, discharge, indemnify and hold harmless Prudent Pet and each of its respective affiliates, subsidiaries, parent corporations, and their officers, directors, employees, representatives and agents from and against any claims made by the participants, entrants or any other parties related in any way to the operation of this Referral Program, as well as any other claims, damages or liability due to any injuries, damages or losses (whether alleged, threatened, or actual) to any person (including death) or property of any kind resulting in whole or in part, directly or indirectly, from acceptance, possession, misuse or use of any prize or participation in any activity or participation in this Referral Program.

PRUDENT PET MAKES NO WARRANTY, EXPRESS, IMPLIED, STATUTORY OR OTHERWISE, WITH RESPECT TO THE REWARD PROGRAM OR ANY PARTICIPATION IN THE REWARD PROGRAM. PRUDENT PET EXPRESSLY DISCLAIMS ALL WARRANTIES EXPRESS OR IMPLIED INCLUDING ALL WARRANTIES OF NONINFRINGEMENT, MERCHANTABILITY, FITNESS FOR ANY PARTICULAR PURPOSE, AND ANY WARRANTIES IMPLIED BY A COURSE OF DEALING OR PERFORMANCE REGARDLESS OF WHETHER EXPRESS OR IMPLIED BY LAW, COURSE OF DEALING, COURSE OF PERFORMANCE, USAGE OF TRADE OR OTHERWISE. PRUDENT PET DOES NOT WARRANT THE DATA, CONTENT, FEATURES, OR INFORMATION PROVIDED IN CONNECTION WITH THE REWARD PROGRAM WILL BE UNINTERRUPTED, ACCURATE, OR FREE OF ERRORS, VIRUSES, OR OTHER HARMFUL COMPONENTS, OR THAT ANY REWARD OR DONATION WILL MEET YOUR EXPECTATIONS.

Privacy Policy:

Information collected, processed and used in connection with this Referral Program may be used by Prudent Pet in accordance with these Terms and the Prudent Pet Privacy Policy.

Other:

Information collected, processed and used in connection with this Referral Program may be used by Prudent Pet in accordance with these Terms and the Prudent Pet Privacy Policy.
You are not authorized to sell, solicit, or negotiate insurance on behalf of Prudent Pet Insurance or its partners.
